Text For Citation: 01 Item/Group: 001 Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a struck-by hazard: a.Employees were exposed to the hazard of being struck by the boom of the Northwest 95 WT crawler dragline while being on Spud Barge GCC Number 102 which was located at Mile 539 1/2 Intracoastal Canal, Ingleside, Texas. The employer did not perform complete annual inspections on a Northwest 95 WT crawler dragline and its associated equipment and maintain written records with a determination made as to whether deficiencies constituted a safety hazard. Among other methods, one feasible and acceptable abatement method to correct this hazard is to implement a complete annual inspection program with dated and signed written inspection records and reports of floating cranes/derricks and its associated equipment as required by American Society of Mechanical Engineers (ASME) Standard ASME B30.5-1994. b.Employees were exposed to the hazard of being struck by the boom of the Northwest 95 WT crawler dragline while being on Spud Barge GCC Number 102 which was located at Mile 539 1/2 Intracoastal Canal, Ingleside, Texas. The employer did not maintain monthly, dated and signed written inspection records and reports on critical items in use such as brakes, crane hooks and ropes on a Northwest 95 WT crawler dragline and its associated equipment with a determination made as to whether deficiencies constituted a safety hazard. Among other methods, one feasible and acceptable abatement method to correct this hazard is to implement a regular monthly inspection program with dated and signed written inspection records and reports on critical items in use such as brakes, crane hooks and ropes as required by American Society of Mechanical Engineers (ASME) Standard ASME B30.5-1994. c.Employees were exposed to the hazard of being struck by the end of the boom of the Northwest 95 WT crawler dragline while being on Spud Barge GCC Number 102 which was located at Mile 539 1/2 Intracoastal Canal, Ingleside, Texas. The employer did not maintain monthly, dated and signed written inspection records and reports on rope condition, i.e. reduction of rope diameter below nominal diameter; number of broken outside wires; worn outside wires; corroded or broken wires at end connections; corroded, cracked, bent, worn or improperly applied end connections and severe kinking, crushing, cutting or unstranding for a Northwest 95 WT crawler dragline and its associated equipment with a determination made as to whether deficiencies constituted a safety hazard. Among other methods, one feasible and acceptable abatement method would be to implement a regular inspection program and maintain monthly, dated and signed written inspection records and reports on ropecondition as required by American Society of Mechanical Engineers (ASME) Standard ASME B30.5-1994.
