29 CFR 1910.178(m)(5)(iii): Powered industrial truck(s), with the operator dismounted and within 25 feet of the truck still in his view, did not have load engaging means fully lowered: a) On or about April 3, 2025, at the jobsite located at 3775 NW 81st St., Miami, FL 33147, employees were exposed to a crush by hazard while having their lower body underneath the elevated portion of the forks on Toyota forklift No. 8 that was holding metal sheet as it was being bent by an AccurPress Press break (Model No. 725014).
