29 CFR 1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury: a) Trench, West side of house: On or about February 20, 2025, the employer exposed employees to engulfment and cave-in hazards, in that; the employer did not instruct employees to recognize and avoid unsafe conditions, such as but not limited to, recognition of warning signs of trench collapse and proper sloping/benching procedures while performing work at site.
