Text For Citation: 01 Item/Group: 001 Hazard:
1910.147(d)(5)(i): Following the application of lockout or tagout devices to energy isolating devices, all potentially hazardous stored or residual energy shall be relieved, disconnected, restrained, and otherwise rendered safe. On or about October 30, 2024, at the facility located at 3935 Westgate Road in Grand Island, Nebraska: The employer failed to develop procedures to control hazardous energy during maintenance or servicing activities on equipment, including but not limited to the Caterpillar 259B3 Compact Track Loader. An employee suffered a fatal injury when the raised and unsecured control arm assembly was released and shifted forward during a maintenance activity.
