29 CFR 1910.132(a): Protective equipment was not used when necessary, whenever hazards capable of causing injury and impairment were encountered. On or about December 2, 2023, and at times prior thereto, employees conducted repair operations to the Auxiliary "A" transformer cooling system, and the employees did not wear protective equipment to protect from electrical hazards to the eyes or face. This condition exposed employees to arc flash and electrical contact hazards.
