29 CFR 1910.23(c)(11): Portable ladders used to gain access to an upper landing surface did not have side rails that extend at least 3 feet (0.9 m) above the upper landing surface: On or about November 12, 2023, employees of Transformer Power Services LLC were commissioning and testing an electrical transformer located at 15141 State Highway 177 in Jackson, Missouri. The employer did not ensure a portable ladder used to access the upper surface of a electrical transformer was positioned with the siderails extending 3 feet above the landing surface. The ladder was placed on the West side of the transformer, the upper surface contained bushings, arrestor brackets, copper bus bars and protruding bolts and nuts which created obstacles for how the ladder could be positioned.
