Alleged Violation Description 29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. Warehouse (Loading Dock): On or about 8/1/2022, an employee was struck by an 18-wheeler truck backing up to the loading dock, causing him to be crushed between the truck and loading dock. The employer did not report the amputation until 8/4/2022.
