29 CFR 1910.22(a)(3): The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice: a.) Ellwood City Forge, LLC dba Ellwood Mill Products - On or about, 2/22/22, an employee was working on the Wagner Lathe when the grating the were standing on slid due to a missing piece of grating and grating not being secured. The grating opened and the employees leg fell into the chip conveyor trough below. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
