29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. (a) At the facility, the employer failed to report by phone, in person at the nearest OSHA area office, or by electronic submission an accident that occurred on February 17, 2022, which resulted in an in-patient hospitalization within twenty-four (24) hours.
