29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: The employer is failing to protect employees from amputation hazards. This was most recently documented on or about March 3rd, 2022, at 1329 West Highway 6, in Holdrege, Nebraska 68949. An employee was visually inspecting an unguarded mechanical system - The employee used a paper towel to assist in checking for burrs and defects in the cable by folding and positioning the paper towel around the three cables of the pulley system. The paper towel caught on the cable, pulling the employees right hand into the pulley resulting in an amputation.
