29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form: The employer failed to evaluate respiratory hazards, including carbon monoxide levels. On or about February 17, 2022, at Hornady Manufacturing - 3625 W. Old Potash Hwy, Grand Island, Nebraska 68803, an employee engaged in conducting inventory in an enclosed warehouse utilizing an LPG forklift, was exposed to high carbon monoxide levels and required hospital treatment for carbon monoxide exposure.
