29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) Ohio Polytech, LLC. at the site location of 201 E. Beal Avenue, Bucyrus, OH: On or about February 13, 2022, the employer did not ensure employees were protected while performing unjamming activities at the Titan Conveyor of the "Shredder" Line. The employer did not ensure that the energy control application steps to control the electrical, hydraulic and gravitational sources were implemented prior to unjamming product materials from the conveyor belts.
