29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: The employer failed to protect employees from exposure to in-running nip points associated with processing cow hides through a Down Puller Machine. This was most recently documented on February 11, 2022, when an employee attempted to free a cow hide that had become stuck between the shield and guard of the Down Puller Machine, and received an amputation injury.
