29 CFR 1910.28(b)(3)(i): The employer did not ensure that each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following methods listed in 29 CFR 1910.28(b)(3)(i)(A)-(D): a) DC6 Platform; On or about January 31, 2022 and at times prior, the employer exposed employees to fall hazards in that employees were allowed to walk and work around a 4 feet by 4 feet square floor hole, approximately 20 feet above the next level, and were not protected by a hole cover, fall restraint system, fall arrest system, or railing
