29 CFR 1910.141(a)(3)(i): Places of employment were not kept clean to the extent that the nature of the work allowed: On or about January 31, 2022, Prolec Transformer Substation PB46 (2500 KVA) located outdoors was not maintained or kept clear of weeds, vegetation, and debris. This condition exposed employees trip hazards and / or the harborage of rodents, insects, and other vermin.
