29 CFR 1910.29(b)(2): The employer did not ensure that guardrail systems met the requirement that midrails, screens, mesh, intermediate vertical members, solid panels, or equivalent intermediate members that are installed between the walking-working surface and the top edge of the guardrail system as follows when there is not a wall or parapet that is at least 21 inches (53 cm) high: On or about January 28, 2022, at jobsite located at 1251 Burlington Street Opa Locka 33054, an employee was exposed to a fall hazard of 8 feet while working from an elevated platform of a cement blender machine whose guardrail lacked a midrail.
