29 CFR 1910.119(f)(1):The employer did not develop and implement written operating procedures that provided clear instructions for safely conducting activities involved in each covered process consistent with the process safety information and that addressed at least the following elements. 29 CFR 1910.119(f)(1)(ii): Operating limits. 29 CFR 1910.119(f)(1)(iii)(A): Properties of, and hazards presented by, the chemicals used in the process. 29 CFR 1910.119(f)(1)(iii)(B): Precautions necessary to prevent exposure, including engineering controls, administrative controls, and personal protective equipment. a) Plant, Test Tunnel for Light Intensity area; on or about January 13, 2022, and at times prior to, employees were exposed to thermal radiation skin burns, in that the employer did not develop and implement an operating procedure for testing/demonstration of the energy content of pyrotechnic materials that addressed at least the maximum safe amounts, the hazardous properties, and the controls on electrical matches to prevent accidental ignition. b) Plant, Test Tunnel for Light Intensity area; on or about January 13, 2022, and at times prior to, employees were exposed to thermal radiation skin burns, in that the employer did not ensure that the light intensity measurement procedure PSI-P-0886 addressed controls on electrical matches to prevent accidental ignition of the samples.
