29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: Employees engaged in maintenance activities at the bone eliminator by the final grinder located in the Toppings Formulation Production area were exposed to amputation, caught-in and/or caught-between hazards in that the employer had not ensured that the air line valve had been locked out prior to an employee placing his hand on the white plastic piston (shaft) mounted on to an air cylinder, resulting in an injury to his hand, amputation of the tip of his finger.
