29 CFR 1910.269(a)(2)(ii) : Qualified Employee Training; Each qualified employee was not trained and competent in sections 1910.269(a)(2)(ii)(A) through 1910.269(a)(2)(ii)(E): Jobsite -35 Willard St. Grafton, MA. Pole 15 On or about 08/20/2021, the employer did not ensure that each electrically qualified employee was trained in, competent in and have the skills and techniques necessary to determine the nominal voltage of exposed live parts, the minimum approach distances specified in this subpart corresponding to the voltages to which the qualified employee will be exposed and the skills and techniques necessary to maintain those distances, the proper use of the special precautionary techniques, personal protective equipment, insulating and shielding materials, and insulated tools for working on or near exposed energized parts of electric equipment, and the recognition of electrical hazards to which the employee may be exposed and the skills and techniques necessary to control or avoid these hazards. Specifically, on 08/20/2021 employees installing a low side cutout lead to the conductor on the road side phase were not aware of how to suffiently cover the exposed live equipment with adequate lengths of protective line hoses and blankets prior to comencing with making low side connections. Additionally, the employees did not position themseves on the outside of the roadside line to minize the arc flash potential posed by the center phase line. Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29CFR1903.19(d)(1).
