29 CFR 1910.212(a)(3)(ii):The point of operation of the machine whose operation exposed an employee to injury, was not guarded. The guarding device was not in conformity with any appropriate standards therefore, or, in the absence of applicable specific standards, will be designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. On or about June 14, 2021 in the bailer room, the point of operation was not guarded. This condition exposed employee to caught between hazards.
