29 CFR 1915.159(b)(5):Horizontal lifelines shall be designed, installed, and used under the supervision of a qualified person, and shall only be used as part of a complete personal fall arrest system that maintains a safety factor of at least two. a) On and about June 22, 2021, on Doug's vessel at the shipyard, a horizontal lifeline was not installed and used as part of a complete fall arrest system to maintain a safety factor of at least two when employees were installing handrails on a scaffolding 14 feet from the ground level.
