29 CFR 1910.217(c)(1)(i):The employer did not provide and ensure the usage of point of operation guards or properly applied point of operation devices on every operation performed on mechanical power press: (a) On June 14, 2021, and at times prior, for the employee's operating a full revolution punch press (serial #6-4711 or company asset 32) where the machine guard allowed access to the point of operation resulting in an employee receiving a partial amputation and crushing injuries to their hand at the plant production area at 2331 North 42nd Street, Grand Forks, North Dakota.
