29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Napkin Machine #5: On or about May, 27, 2021, employees were exposed to amputation hazards when they by-passed an interlock to re-thread paper in-between the rollers without developing and implementing a lockout tagout procedure.
