29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips, and sparks. Examples of guarding methods are barrier guards, two-hand tripping devices, electronic safety devices, etc. a) On or about May 20, 2021, the employer did not ensure that machine guarding was provided on the Gamma portable winding machine exposing employees to a caught by hazard created from ingoing nip points created by the plastic film being rolled onto spindles and the rotational hazard of the unused portions of the spindles on the front and back sides of the machine b) On or about May 24, 2021, the employer did not ensure that machine guarding was provided on the winding machine line 13 exposing employees to a caught by hazards from ingoing nip points created by rollers feeding plastic film on the front of the machine and from ingoing nip points of the plastic film being rolled onto spindles and the rotational hazard of the unused portions of the spindles on the back and sides of the machine. c) On or about May 24, 2021, the employer did not ensure that machine guarding was provided on the SANO winding unit number 5, exposing employees to a caught by hazard created from ingoing nip points of the plastic film being rolled onto spindles and the rotational hazard of the unused portions of the spindles on the front and back and sides of the machine. d) On or about May 24, 2021, the employer did not ensure that machine guarding was provided on the SANO winding unit number 6, exposing employees to a caught by hazard created from ingoing nip points of the plastic film being rolled onto spindles and the rotational hazard of the unused portions of the spindles on the front and back and sides of the machine. e) On or about May 24, 2021, the employer did not ensure that machine guarding was provided on the SANO winding unit number 7, exposing employees to a caught by hazard created from ingoing nip points of the plastic film being rolled onto spindles and the rotational hazard of the unused portions of the spindles on the front and back and sides of the machine. f) On or about May 24, 2021, the employer did not ensure that machine guarding was provided on the SANO winding unit number 8, exposing employees to a caught by hazard created from ingoing nip points of the plastic film being rolled onto spindles and the rotational hazard of the unused portions of the spindles on the front and back and sides of the machine. g) On or about May 24, 2021, the employer did not ensure that machine guarding was provided on the B-3 Bell-Mark Inline Printer exposing employees to a caught by hazards from ingoing nip points created on top of the machine by the printing drum and the frame. h) On or about May 24, 2021, the employer did not ensure that machine guarding was provided on the B-4 Bell-Mark Inline Printer exposing employees to a caught by hazards from ingoing nip points created by rollers on top of the machine feeding plastic film. i) On or about May 24, 2021, the employer did not ensure that machine guarding was provided on the Battenfeld Gloucester winding machine 144 on line 11, exposing employees to pinch points on the front of the machine during indexing and, caught by hazard created from ingoing nip points of the plastic film being rolled onto spindles and the rotational hazard of the unused portions of the spindles on the front back and sides of the machine. j) On or about May 24, 2021, the employer did not ensure that machine guarding was provided on the winding machine 192 D on line 10, exposing employees to a caught by hazard created from ingoing nip points of the plastic film being rolled onto spindles and the rotational hazard of the unused portions of the spindles on the front side of the machine.
