29 CFR 1910.29(b)(2):The employer did not ensure that midrails, screens, mesh, intermediate vertical members, solid panels, or equivalent intermediate members were installed between the walking-working surface and the top edge of the guardrail system when there was not a wall or parapet at least 21 inches (53 cm) high: On or about April 27, 2021, an employee was exposed to fall hazards above fifteen feet when positioned on top of the Special Purpose Personnel Elevator (SPPE) and the employer did not ensure the guardrail system met the requirements of the standard.
