29 CFR 1910.147(c)(4)(ii)(B): Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; a. 2115 Industrial Blvd. Cornelia, GA (Myen Machine #1 Elevator Belt) - An employees were exposed to struck-by and/or caught between hazard of the single energy source motor shaft/gear box by not unplugging the cord before servicing (sanding) the motor shaft/gearbox which can cause sudden energization or start-up of the single energy source apparatus (motor shaft/gear box). The employer did not incorporate machine specific procedure for the single energy source motor shaft/gear box (cord and plug) into the company's existing written lockout/tagout procedures for the Myen Machines. An employee did not disconnect/unplug the motor shaft/gearbox which is equivalent to lockout/tagout before servicing/maintenance, on or about 03/23/2021. Note: The exception to 29 CFR 1910.147(c)(4)(i) was not followed in its entirety.
