OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to caught-in, crushing, cutting and amputation type injuries while manually removing a shaft from a cylinder by striking it against a hard surface: (a) Maintenance Area: On or about 3/29/2021, employees rammed a metal roller into a forklift counterweight as a means of dislodging the roller shaft from the cylinder, which resulted in a finger amputation injury to an employee performing this task.
