29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: a. On or about 1:00 AM on February 12, 2021, at 2525 First Street, Fort Myers, FL, 33901, an employee suffered a work related injury and was hospitalized for treatment in the morning of February 12, 2021. The employer was aware of the in-patient hospitalization in the morning of February 12, 2021. The employer notified OSHA on March 3, 2021 at 4:08 PM of the in-patient hospitalization.
