29 CFR 1926.502(d)(16)(iii):Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: Hector Hernandez Sr. at 1600 Ingalls St. Lakewood, CO 80214: On or before 1/13/21, the correcting, creating, and exposing employer did not protect employees from injury in that two employees who were tied off fell approximately 15 feet and struck the ground. One employee was hospitalized as a result. This condition exposed employees to a 15 foot fall hazard. Abatement Note: Abatement certification and documentation are required for this item.
