29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) On or about October 29, 2020 at the worksite located at 903-A GA Hwy 124 in Braselton, Georgia the employer failed to report to OSHA a work-related hospitalization of an employee within 24 hours. b) On or about November 22, 2020 at the worksite located at 903-A GA Hwy 124 in Braselton, Georgia the employer failed to report to OSHA a work-related hospitalization of an employee within 24 hours. Petco Animal Supplies Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1904.39(a)(2), which was contained in OSHA inspection number #1298360, citation number 1, item number 1 and was affirmed as a final order on August 30, 2018, with respect to a workplace located at 257 Prospect Plains Rd, Cranbury, NJ 08512 .
