29 CFR 1910.178(a)(4):Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) Warehouse Interior: On or about 11/23/2020, and at times prior, the employer exposed employees to struck-by hazards, in that boom attachment (Model# 4WYT3, SER# 1210-057455-42-23) was being used without the prior approval of the manufacturer. The attachment was used to unload and move materials throughout the warehouse.
