Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.147(a)(2)(ii)(A):Lockout/tagout procedures were not implemented when servicing and/or maintenance was taking place during normal production operations when an employee (s) was required to (A) Remove or bypass a guard or other safety device; (B) were required to place any part of his or her body into area on a machine or piece of equipment where work was actually performed upon the material being processed (point of operation) or where an associated danger zone existed during a machine operating cycle: Employees were exposed to amputation, caught-in, and struck-by hazards. Employees engaged in maintenance activities on mixer number 8, were exposed to amputation, caught-in, and struck-by hazards in that Lockout/tagout procedures were not implemented when maintenance was taking place during normal production operations. Employees were bypassing the emergency stop when performing regular maintenance on mixer number 8.
