29 CFR 1910.178(o)(1): The employer did not ensure that only stable or safely arranged loads were handled: a) National Grid Yard - 7437 Henry Clay Blvd, Liverpool, NY: On or about 11/5/20, two employees, one operator and one spotter on the ground, were using a JCB 512-56 Tele-handler all terrain forklift to load 20 ft. sections of monopole towers, weighing approximately 8,000 lbs. each, onto a flatbed trailer. The tower sections were not uniform in size or shape causing the load to de-stabilize and fall off the trailer when the operator attempted to stack them, striking the employee on the ground and resulting in fatal injuries. The employer did not ensure that the tower sections were stable or safely arranged while being loaded.
