29 CFR 1910.134(e)(2)(i):The employer did not identify a physician or other licensed health care professional (PLHCP) to perform medical evaluations using a medical questionnaire or an initial medical examination that obtained the same information as the medical questionnaire: (a) At the facility employees are required to wear 3M brand Full Face respirators while performing work with cement were exposed to respiratory hazards in that the employer failed to ensure that PLHCP performed medical evaluations prior to use.
