29 CFR 1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) St. Henry Tile Co. Inc, on or about October 16th, 2021, at the location of 38 S Webster St., Minster, OH, 45865: The employer did not ensure that employees received training specific to the equipment utilized, a 2019 Terex Front-Discharge Mixer , exposing employees to potential for injury associated with unsafe use of the equipment. An employee suffered an amputation of a fingertip while washing out the mixing drum of this truck.
