29 CFR 1910.147(a)(2)(ii)(B): (A) An employee was required to place a part of his or her body into an area on a machine or piece of equipment where work was actually performed upon the material being processed (point of operation) or where an associated danger zone existed during a machine operating cycle. A blow mold process technician employee conducting maintenance activities on Airlock #65 was exposed to caught-in and amputation hazard, in that Grinder #65 and Airlock #65 was not de-energized prior to reaching into the Airlock #65 to clear re-grind material.
