1926.454(a)(5) : The employer did not have each employee who performs work on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards, including any other pertinent requirements of this subpart: On or about September 1, 2020, at 7960 Johnson Street, Pembroke Pines, Florida, employees not trained in all pertinent requirements pursuant to this standard, were exposed to struck-by and tip-over/fall hazards while engaged in the task of removing slurry inside an accelerator tank.
