29 CFR 1910.29(f)(4): Openings in a stair rail system exceeded 19 inches at its least dimension: Employees monitoring grain levels in the south mill were exposed to fall hazards of up to approximately 8 feet from the walking surface to the ground below in that the stair rail system, installed prior to January 17, 2017, meeting the pre-January 17, 2017 top rail height requirements for a handrail/stair rail combination, was not equipped with a rail, was missing a rail on the left side and approximately 60 inch of the rail was missing on the right side
