29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): Or in the alternative: 29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Fabrication Shop - During the period of March 1, 2020 through August 6, 2020 the employer allowed employee(s) to tape carpet on Bertsch plate roll machines while the machines operated without machine guarding exposing employees to amputation hazards created by in going nip points of rotating parts. Control levers were taped down in a manner that allowed the machines to run without an employee operating the control during the process of taping the rolls.
