29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. An In-patient hospitalization affecting one (1) employee, occurring on or about 6/19/20, was not reported to the local OSHA area office within the 24-hour reporting requirement. Southwestern Telephone Company-AT&T- Oklahoma was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1904.39(a)(2), which was contained in OSHA inspection number 1258242, citation number 1, item number 1and was affirmed as a final order on 1/18/18, with respect to a workplace located at 710 Terry Land, Richardson, TX. 75080.
