29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) J&P Roofing at 1891 E 68th Ave. Denver, CO 80229: On or before 6/24/20 the employer did not protect employees from injury in that employees using the metal roofing panel extruder [New Tech Machinery SSQ SN#8390718] were not protected from amputation hazards where the shear was located. An employee lost three fingers from one hand performing this task. This condition exposed employees to amputation hazards.
