29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about 3/13/2020 a machine operator was attempting to free a jam of the chute door between bins # 3 and #4. The jam was cleared, which caused the door to close automatically on the employee's arm fracturing it.
