Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards during manual gantry crane movement with defective and malfunctioning caster wheels: On or about February 19, 2020, at D&S Distribution Inc. 17000 Rockside Road, Maple Heights, OH, 44137 the employer moved a Vestil FHS 4-20-Series Steel Fixed Height Gantry Crane with defective caster wheels. Employees were exposed to serious injuries while moving the gantry crane and the caster wheels were not operating properly and the upper beam fell due to not being properly attached to the A-frame supports.
