29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: (a) Jobsite, WB-454 Bridge Replacement, Pittsburgh, PA: On or about November 10, 2019 an employee working from an aerial lift was not tied off to the boom or basket. The employee fell from the lift and sustained a broken pelvis and fractures to both wrists.
