29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources: a). Feed Department, Sand Dump Area - On or about and times prior to November 4, 2019, the employer did not provide isolating device equipment (e.g. locks, tags) for feed operators to isolate and secure a vacuum system's energy source during the installation of a dust collection bag onto the discharge opening of the vacuum system, thus exposing employees to a caught-in hazard.
