29 CFR 1910.147(f)(2)(i): When outside servicing personnel were engaged in activities in a facility, the onsite employer and the outside employer did not inform each other of their respective lockout or tagout procedures: a) On or about 10/14/2019, Employer had not informed the contractor of the Lockout/Tagout (Energy Control) procedures for the #12 overhead door, Industrial Duty Door Operator, prior to the contractors aerial work platform being hit by the overhead door which was opened remotely while the contractors employee was working in the elevated aerial platform. This was also in violation of the companies own policies which are outlined in their Lockout/Tagout (Energy Control) Written program 3.3. 52.1.3, Section 6.12 Contractor Information Exchange. b) On or about 10/28/2019, Employer had not informed the contractor of the Lockout/Tagout (Energy Control) procedures for the Bay 9 overhead door, Industrial Duty Door Operator, prior to the contractors aerial work platform being hit by the overhead door which was opened remotely while the contractors employee was working in the elevated aerial work platform. This was also in violation of the companies own policies which are outlined in their Lockout/Tagout (Energy Control) Written program 3.3. 52.1.3, Section 6.12 Contractor Information Exchange.
