Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): A.) JM Manufacturing Company, Inc. dba JM Eagle; PVC Mixer Building - On or about 10/28/19, and at times prior thereto, employees were exposed to hazardous energy sources while performing work on a drop tube for a mixer. The valve on the drop tube was not locked out and/or tagged out when an employee was attempting to clear the tube. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
