29 CFR 1910.106(e)(6)(i): Adequate precautions were not taken to prevent the ignition of flammable vapors: A.) Lindy Paving, Inc.; 3580 Wheelertown Rd., Waterford, PA - On or about October 1, 2019, and at times prior thereto, diesel was being used to clean the drag conveyor. Vapors were able to accumulate inside the enclosed conveying system. A flash fire occurred in the conveying system and burned an employee. NO ABATEMENT CERTIFICATION OR DOCUMENATION REQUIRED
