29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. On September 19, 2019, at approximately 8:10 PM, an employee suffered from amputation of two fingertips on the right hand which additionally required in-patient hospitalization. The amputation and hospitalization was not reported within 24 hours to the local area office of the Occupational Safety and Health Administration. The Austin Area Office was notified on September 30, 2019 at 9:44 AM, by an employer representative.
