29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: On or about June 24, 2019, at 1494 Northlake Way, Palm Beach, Florida, 33480, an employee who was not trained in the recognition and avoidance of unsafe conditions related with the operation and clean-out of a cement pump hopper, was exposed to an amputation hazard, while removing concrete from the hopper with the use of his hand. NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
